Don't know what to do next?
You shouldn't have to figure it all out alone. We help families understand their options, find appropriate resources and coordinate the next step — and we stay with it until something actually happens.
Five questions to start
Answer in your own words. Please leave out medical records and diagnoses — a coordinator will collect anything clinical properly, with your consent, later.
How it works
- 01
You describe the situation.
- 02
A coordinator reviews it and calls you.
- 03
You get real options and a written next step.
- 04
We follow up until the next step is done.
What we don't do
- We do not diagnose or give medical advice.
- We do not determine medical necessity.
- We do not decide for you — you choose the provider.
- We do not present a resource as verified when it isn't.
What families ask us about
- Aging and caregiver support
- Disability support
- Coming home from the hospital
- Transportation and meals
- Community resources and benefits
A referral is not a phone number
We don't count a referral as done when we hand you a number. We send it, confirm it was received, check whether the service was accepted and scheduled, then follow up and record the outcome.
If this is a medical emergency, call 911 or seek emergency medical care. Tender Loving Care of Hawaii is not an emergency medical service.
