What a referral includes
- Referral source and your contact details
- Patient or family contact information
- Only the information you are authorized to share
- Requested service and urgency
- Notes relevant to coordination
- Consent or authorization documentation where applicable
Anonymous submissions containing patient health information are not accepted. Every referral is attributed to an identified, authorized submitter.
How it works

A coordinator reviews it and confirms receipt with you.

Assessment and option development begin with the family.

You receive status updates on referrals you submitted.
What partners can and cannot see
You can see
- Referrals your organization submitted
- Their current coordination status
- Follow-up milestones relevant to your referral
You cannot see
- Referrals submitted by any other partner organization
- Patient records unrelated to your referral
- Workforce, facility or internal administrative records
