For families

Finding Care Doesn't Have to Feel Overwhelming.

A care coordinator will listen to your situation, help you understand the options that exist, and stay with you through the next steps.

What we help families navigate

  • Assisted living
  • Board and care homes
  • Home-care options
  • Skilled nursing and convalescent care
  • Post-hospital transitions

What a coordinator asks about

  • Day-to-day functional status and mobility
  • Help needed with daily activities
  • Supervision and memory support needs
  • Medication support
  • Safety concerns at home
  • Preferences, budget, island and neighborhood
  • How soon a decision is needed

How it works

  1. 01

    Tell us what you need.

  2. 02

A coordinator completes an assessment with you.

  3. 03

You receive care-setting guidance and provider options.

  4. 04

You choose — and we coordinate and follow up.

The choice is always yours

We present options. We do not decide for you. Every option we share, every question you ask, and the provider you ultimately select are documented so the process stays clear to everyone involved — including you.

After a move-in, a coordinator follows up at 24 hours, 7 days and 30 days to check that things are going the way you expected.

What we don't do

  • We do not diagnose or provide medical advice.
  • We do not determine medical necessity or clinical suitability.
  • We do not make a placement decision on your behalf.
  • We do not select providers based on what a provider pays.

If this is a medical emergency, call 911 or seek emergency medical care. Tender Loving Care of Hawaii is not an emergency medical service.
